Skeletal variations and dental profiles are closely related to skeletal class III malocclusion, with maxillary hypoplasia and mandibular hyperplasia, or a combination of both. Many factors affect facial asymmetry including condylar hyperplasia, condylar fracture, condylar resorption, internal bleeding of the temporomandibular joint (TMJ), and a history of one-sided chewing. The standard approach for class III treatment is a combination of pre-surgical and post-surgical orthodontic treatment, accompanied by orthognathic surgery. The article presents of 31 years old male with class III malocclussion and asymmetric mandible who underwent combined treatment of orthodontic and orthognatic surgery to solve his malocclusion and improved aesthetics and functional occlusion. Decompensation orthodontic phase involved in preadjusted edgewise orthodontic, with MBT prescription. The patient underwent orthognatic surgery to correct maxilla, mandible prognatism, and menton deviation. Post surgical with orthodontic treatment was accomplished for finishing stage. The patient's aesthetics and functional occlusion were improved. Performing treatment in three phases achieve better occlusion stability after surgery, appropriate dental compensation, jaw coordination, and obtain more predictable results before orthognathic surgery. © (2024), (University of Dicle).
