Understanding adherence to program therapy in people with schizophrenia: A comprehensive concept analysis

Abstrak
Understanding adherence to program therapy in people with schizophrenia is crucial for improving treatment outcomes and preventing relapse, a major challenge in managing this condition. Schizophrenia remains a significant global mental health issue, affecting millions worldwide, with non-adherence to therapy often leading to worsening symptoms, hospitalization, and decreased quality of life. Adherence plays a vital role in ensuring treatment effectiveness, enhancing patients' overall well-being, and supporting long-term stability, yet complex psychological, social, and systemic factors influence it. This study employs Walker and Avant’s concept analysis framework to examine adherence in individuals with schizophrenia, systematically defining the concept, identifying its attributes, antecedents, consequences, and empirical referents to provide a structured understanding applicable in clinical practice. The findings highlight four key attributes of adherence: commitment, which refers to patients' determination to follow therapeutic recommendations; program therapy, encompassing medication, psychosocial interventions, lifestyle modifications, and diet adherence; healthcare workers, whose guidance, monitoring, and support are essential in reinforcing adherence and ensuring its continuity; and active patient participation, emphasizing their role in managing their condition and maintaining treatment engagement. Key antecedents, including patient education, social support, financial accessibility, and a strong therapeutic alliance, significantly impact adherence. Consequences include improved symptom management, reduced relapse rates, enhanced functionality, and better integration into daily life. Based on these results, concept analysis showed the conceptualization of meaning as a significant adherence to program therapy among people with schizophrenia. By understanding the core concept of adherence, healthcare workers can provide the right care for people with schizophrenia, which will enhance adherence and prevent relapses. A clear understanding of adherence helps healthcare professionals develop effective interventions, improving patient commitment and outcomes. Addressing key factors enhances therapy engagement, reducing relapses. Strengthening patient-centered care, awareness, and collaboration further boosts adherence, making treatment more sustainable. © 2026, Malque Publishing. All rights reserved.
