Objective: This case report aims to describe and present a unique or intriguing clinical scenario involving the misalignment and poor healing of several fractures in the mandible that were left untreated or undertreated. Case Report: A 22-year-old woman complained of biting and chewing discomfort following ORIF surgery two months earlier. Previously, the patient had suffered multiple fractures to her face and jaw due to a motorbike accident. Clinical and radiological examination revealed a neglected right mandibular ramus fracture and malunion of the mandibular parasymphysis and body fracture. The patient was successfully treated by removing the old plates, refracturing, re-plating, and applying intermaxillary fixation. Upon recall, the patient did not complain of chewing or biting difficulty, malocclu-sion, pain, or facial asymmetry. Results: Malunion is one of the complications following ORIF. In this case, malunion results from insufficient reduction and inappropriate plate selection. The treatment successfully eliminated the patient's main complaint. It also corrected malocclusion, one of the main symptoms of malunion. Conclusion: In this instance, a combination of insufficient fracture reduction, inappropriate selection of the fixation plate, and the failure to address a fracture resulted in several malunions. Fortunately, our team successfully resolved the issue without significant complications. This treatment protocol may offer a valuable approach to managing comparable cases. © 2025 JDMFS.
